  【摘要】：(續2):多方面綜合論從盧致鵬博士教授治療腫瘤綜合治療的臨床經驗體會用法思緖等方法規納當中,吸取其辨證思維套緒、綜合治療原則、治療模式機理等內容、概念基本原則、治療條件和存在因素等探討、用法論證、理論歸原、治方大法、問源求根、臨證用方等獨特治療原則,得出如下經驗體會: 腫瘤方面治病必求於本,思緒宜遵腫瘤治療原則等方面的生理病理原則; 確立正確的治療原則方式,並配合中西醫結合方式,循序進行或臨床模式必求之中西醫各自的優點,擇而配治; 注重過程,明確目的,不可盲目亂治或雜亂無章,自阻腳步,安排要合理,以確保腫瘤過程規律化及程式化; 縮小局限腫瘤部位及範圍,阻斷腫瘤傳播異化及趨勢,乃為治療腫瘤惡化或異變的主要方法與途徑;確立腫瘤治療的基本原則,配合中西醫結合方式著手;明局部與整體並重的治療程式原則,以務求腫瘤的徹底控制或療癒思維;明確腫瘤分期治療的思維和方向,藉中西結合綜合處理為方針用藥趨勢;確立個體化治療思慮原則,以應臨床診治辨証施治之根據…等重要指導治療原則。--(待續2)--

[Abstract]: (1): on the comprehensive aspects from the clinical experience of comprehensive treatment of Dr. Lu Zhipeng professor of treatment of tumors in the usage of mind method adapting, absorbing its dialectical thinking set. Comprehensive treatment principle, treatment mode and mechanism of content, the basic principles of the concept, study, treatment conditions and factors. The theory of argumentation, belong to the original prescription, Dafa, ask the source root, etc. the unique principle of treatment with clinical experience, the following experience: the tumor will seek medical treatment in the mind should be physiological and pathological principles such as the principle of tumor treatment compliance; establish the correct treatment principle, and with the combination of traditional Chinese and Western medicine, sequential for Chinese and Western medicine or clinical pattern will seek their respective advantages, and selective treatment; paying attention to the process, a clear objective, not blind chaos or out of order, self resistance footsteps, to reasonable arrangements, to ensure that the procedures and rules during the process of tumor; localize the tumor location and scope, blocking the alienation and trend of tumor spread is swollen. 盧致鵬教授是廣州中醫藥大學、美國國際醫藥大學、香港中醫藥學院等地教授、主任醫生、博士生導師，長期從事於教學及科研臨床等工作,目前從醫已逾35年，善專於腫瘤疾病療治,用法思緒頗具獨特,經驗療治別樹一格,尤以腫瘤疾病及其治療原則方式和用藥模式等內容研究之頗析臨證,更為精到,收功亦為宏偉,名傳遠近同林之輩,筆者們有幸隨師侍診，自感獲益良多，故今將其治療經驗思緒用法原則及有關傳承內容,逐一點敘如下:(續2)----------

【五】:確立腫瘤治療的基本原則,配合中西醫結合方式著手:

盧博士教授常謂要治療腫的過程中,必須首要瞭解治療腫瘤的基本原則,不要一見到腫瘤就以包醫包診等術語來欺騙大眾,須知腫瘤的治療中最少應達到如下目標:如1 延長無病生存期和總生存期; 2 使治療相關的不良反應盡可縮化,而不可包治一定縮小或消化 ; 3 宜與現代化科技相關技術配合化,與瘤共存的觀念必須建立化,以求得生存質量的獲得提高化 ; 4 充分考慮到病人及家屬的牛活條件,盡量的符合成本效益等之原則化 ; 5 宜多方配合中西醫醫生互相討論治療的心得化以作為提高生命力的優質化 ; 6 償試中西藥與中藥植物等方劑的配合治療化..等等方式,以求得各方面的滿意地步 。盧博士教授在從事治療腫瘤病人時,亦常與告誡學生們,要秉持著醫者父母心的心態去行醫,對於病人的腫瘤問題及發現,盡可能的使用各種方法或檢測,以求得腫瘤疾病的早期發現早期治療,使得腫瘤能在發生浸潤前的狀態,能被早期發現,則可以從中找到根治的辦法。早期發現早期治療,也是醫生治療腫瘤的最基本原則。盧博士教授,在此階段治療時,亦會用各種的中西醫方法,包活水煎藥物及中西藥的配合使用,在在都是要確腫瘤疾病的不被惡化及嚴重化,所以盧教授秉持著,寧可錯過也不要放過的治療原則,來跟腫瘤疾病作長期的奮鬥及治療,使得病人能早日從腫瘤疾病的惡夢中重新站起來,以迎向新的生活。盧博士教授常用如下方劑合方以作為治療抗腫瘤的前趨手段和方藥:如採用 萆薢消毒抗癌丹加減方 土茯苓龍膽清肝湯加減方 清肝去癰消癭丹加減方 白花蛇舌草抗毒丹加減方 半枝蓮金銀消毒丹加減方 三稜大黃消腫清肝湯加減方 海帶浙貝消癭丹加減方 消腫散結去瘀湯加減方 蘚砂射幹白花丹加減方..等方劑,參而酌用之,以求腫瘤浸潤擴期前的階段有所改或縮少病灶,以為後一階段的治療作準備。

【六】:明局部與整體並重的治療程式原則,以務求腫瘤的徹底控制或療癒思維

  盧教授認為要治療腫瘤病人方案時,要在重視局部治療的同時,亦要要整體對病性進行分析及預療,要兼顧到腫瘤對全身的影響,並施以相應的治療措施。以全身治療為主,輔以局部治療,往往能得到事半功倍的效果。因為盧教授認為大部份的腫瘤均由局部而引起,然後逐部擴散到全身的,然而腫瘤的發生,與病人的基因缺陷、全身疾病基礎如免疫和內分泌功能的改變有相當關係。在治療腫瘤的同時,必要秉持著全體與局部治療的大思維概念,方能運用自如,亦才能有所對腫瘤的進一步治療與控制的能力,盧教授認為現代治療腫瘤時,腫瘤不完全切除,乃為常事,如作為一種局部治療不徹底的情況之下,加用放療處理局病變和恰當使用全身治療乃為同時重要的,而非僅採一種單方法治療就可控制,必要時,要全身和局部治療同用,且中藥與要藥亦同時交替使用,方能有所成功率。盧教授在臨床上,即常以病人機體作一個整體來看,把病人的病因病灶病機病況逐一作詳細瞭解及探討外,尚須對病人的內在因素如飲食習慣生活細節及綜合習征等作一徹底外,尚須對病人的內因外因不內外因等因素,作一綜合判斷考慮,方能決定一個治療的腫瘤方案,該用手術就用手術,該用放療化療者,就用放療化療,該用中藥調理者,就用中藥調理,多方配合治療,務求使腫瘤的徹底控制消減或逐漸細小化,乃為首要目標。盧教授在治療腫瘤的嚴重發作擴展期,則以西醫手術為主,切除腫瘤部份,之後,再以中醫視其部位辨証然後加以治療,在未得腫瘤徹底控制前,皆以西醫方式治療,然後輔以中醫調理,往往能取得良效,且在腫瘤病灶縮後,往往以中醫調理為主方,配合預後療治,則收效頗佳,且能以姑息療法方式,與瘤共存,以增加和提升病人的生存質量。

【七】:明確腫瘤分期治療的思維和方向,藉中西結合綜合處理為方針用藥趨勢

盧博士教授認為任何的疾病發展過程中,皆有其發病的根據和理由,必當以疾病的來源、轉歸、分析、歸納、預後、處理過程、及其他的病變演進過程,故腫瘤亦不例外,應求其病症的各項分析及解剖,以應相對的治療和對策。在此原則下,盧博士教授常應病人腫瘤的來源及各方面的成因剖析,作一完整的方案,即要以腫瘤的分期來作治療思慮及方式著手,方不致有所延誤病情之譏。盧教授常以疾病的分期來作一中西醫結合處理疾病的方法,即以I期乳腺癌來說,可以採用單純切除手術加放療化療,但I期的非小細胞肺癌,則以根治性肺葉切除為主,術後再輔以提高免疫力的全身治療加上中醫的分型辨証處理,療效可佳。而同是非小細胞肺癌,不冋分期的治療策略亦不相冋,I、II期以手術為主,IIIA期推崇以誘導後手術或放療的模式,IIIB和IV期則以非手術治療為主,而各種分型之西醫治療,亦應配合相對應的中醫分型辨証分型來論治及預療,才能建功,亦使病人早日達到延長生命和生活質量提升的目的。而目前國內外的各種條件及環境,並不一致,如何使國內外的分型分期治療達成一致,乃為現代中西醫界各分子應該努力的重要目標之一。

【八】:確立個體化治療思慮原則,以應臨床診治辨証施治之根據

醫生應根據病人腫瘤的異質性、歸展性、外擴性、易植性、侵入性等方面,及應思考病人之機體功能、心理建設、與體質的陰陽屬性和八綱屬性及其他相應的藥物屬性、物理因素、化學因素等多方面詳加考慮,以確立病人個體化的獨特方案治療方式,方為一個妥善的治療方法和手段。任何一個腫瘤的病例,絶非完全相同,且其病因及轉機歸制,亦不相同,在此情況下所採用的治療方法,當然各有分別,尤其以中年人和老年人同得到腫瘤的治療方式,即亦不同,所採的方法方藥,當然亦皆有別,切不可一見到腫瘤病人,皆採相同的手術或化療放療等千篇一律的治療手段,這亦說明了確立個體化的治療方式,乃為現今醫者所必須學習和適應的重要客題。盧博士教授亦常根據病人的分期外,尚根據病人的體質部位和中醫分型等方式,下去著手治療方案,以求得最合乎病人的最佳療效及生命質量的提升,而非僅以手術或放療或化療為唯一的治療方式,並要求與中醫西醫的配合方式下去著手,分清主次,先急者手術,後再輔以中醫方藥,兩相配合,才能取得良效。在此原則下,盧教授所採的方藥,每人皆不相同,然大原則理論則為相似,則病人的恢復能力及生命亦可適當延長之,此亦乃盧博士教授的最終目的。

另外,盧博士教授亦常在分期辨証的原則下,另加以如下方劑加減,以求其個體之免疫能力的提升,如採用:補中益氣還元丹加減方 參茸白花龍膽湯加減方 承氣去瘀解毒散加減方 大還元養氣散加減方 大小定風珠加減方 天麻雞金丹湯加減方 去瘀養血袪風湯加減 大小承氣湯加減方 連茱龍膽湯加減方 去痺養榮化瘀湯加減方 化膿去瘀養血丹加減方 解毒梔參湯加減方 抗癌消腫化瘀湯加減方..等等方劑,酌而參配治,亦皆取得良效。當然這些方劑應用,亦應經醫生們臨床見症加減而實際運用之,才為恰當。----(待續2)--
